A mild twinge can wait a day or two, but toothache that throbs, keeps you awake or comes with a swollen gum or cheek needs a dentist the same day or the next, because an abscess does not go away on its own. How urgent is your toothache?

Toothache is common: a 2026 meta-analysis of 48 studies found it in about 24% of adults worldwide, and a holiday does not change the rules. The NHS advises seeing a dentist if it lasts more than 2 days, does not go away with painkillers, or comes with a high temperature, pain when you bite, red gums, a bad taste or a swollen cheek or jaw.

What can you do tonight, and who answers at night or on a Sunday?

Rinse gently with warm salty water (half a teaspoon of salt in a glass), eat soft food such as yoghurt, avoid sweet, very hot and very cold things and chewing on the sore side, and do not smoke. Sleep with your head raised, hold something cool against the cheek from outside, never heat, and never put an aspirin on the gum: it burns the tissue.

Which painkiller is sensible, and what does it not do?

The NHS names ibuprofen and paracetamol for toothache, taken exactly as the packet says, and a pharmacist (eczane in Turkey) can advise. Because ibuprofen has more side effects, the NHS suggests trying paracetamol first if you can take it. Ibuprofen does not suit everyone, for example with a stomach ulcer, asthma, a heart, liver or kidney condition, or pregnancy. A higher dose does not work better and can be dangerous, and the NHS says not to use paracetamol for more than 3 days unless a doctor tells you to.

Painkillers dull the pain, they do not remove its cause, and neither do antibiotics for most toothache. The American Dental Association's 2019 guideline found likely negligible benefit and potentially large harms, recommends antibiotics mainly with fever or other signs of spreading illness, and puts treatment of the tooth first.

When is it a hospital case, not a dentist?

Go to a hospital emergency department (acil servis) and do not wait for clinic hours if toothache comes with swelling around the eye or neck, swelling that makes it hard to breathe, swallow or speak, difficulty opening your mouth, a swollen or painful eye, or a high temperature with a swelling that keeps growing. The NHS lists these as emergencies. The ambulance number in Turkey is 112, as the UK Foreign Office advises. Ask hotel reception for help and do not drive yourself if you feel unwell.

Take your passport, your insurer's emergency number and your medicines. The emergency department deals with the danger; the tooth still needs a dentist afterwards, and we can pick it up from there. This is the one case where our clinic is not the right first stop.

How does emergency treatment usually go, and what can wait until home?

The first visit is about stopping the pain. You are examined, told what is causing it and what the options are, and where it can be done safely the urgent part is dealt with at that visit: an aesthetic filling takes 30‑90 minutes, and root canal treatment takes 1‑2 appointments depending on the tooth. For an abscess the NHS describes the dentist draining the pus, then root canal treatment or removing the tooth under local anaesthetic.

Then you decide what must be done now and what can wait. A crown or veneer, if needed, takes 5‑7 days over 2 appointments here, or you can take the written plan to your own dentist at home. Flights matter too: cabin pressure can make an inflamed tooth hurt more, and a 2023 review suggested minimum gaps of 24 hours after a filling and 72 hours after root canal treatment, as a starting point only. Will your travel insurance pay for it?

That depends on your policy, so check it before you pay: whether dental emergencies are covered and up to what limit, whether you must phone the insurer's helpline first, and which documents it wants. The Foreign Office advises contacting your insurer promptly if you are referred to a medical facility. Keep the receipt and ask for a written report of what was wrong and what was done. Whether and how your insurer pays is between you and them. UK visitors should know that the Foreign Office states the EHIC and GHIC are not valid in Turkey, so private travel insurance is what counts.

Message early, not on the last day: chairs are closed on Sunday and a flight date narrows what can be done.
